  • Coping with Rosacea: Tips for Managing Flare-Ups

    Rosacea is a chronic (long-standing)  inflammatory skin condition that causes patchy pink spots, redness of the face, pus-filled pimples, and visible blood vessels extending from nose to cheek. When it worsens, it can spread to your back and chest too. And to top it all, some flare-ups occur with rosacea occasionally, especially with the triggers. Around 5.5% of people worldwide live with rosacea; according to the American Academy of Dermatology, multiple factors play a role in worsening the condition. 

    Some of the triggers can be exposed to the sun, wind, extreme climatic conditions (very hot or cold weather), strenuous exercise, stress, medications causing dilatation of the blood vessels, chemicals in the makeup, spicy food, alcohol, etc. These increase the blood flow to the skin, making the condition worse. Dermatologists recommend that, along with the medications for treating rosacea, patients must be aware of the triggers. It is advisable to keep a journal of what you eat and drink regularly, the shampoo, creams and make-up you use, the daily activities, etc., which can cause flaring up of your rosacea. If you have identified the triggers, avoiding them and managing flare-ups becomes easy.

    Some tips for managing flare-ups are:

    1. Protect your skin from the sun

    If you have rosacea, it is surprising to know that a few minutes of exposure to sunlight on your skin can cause distress (redness, flushing, etc.) and be a cause of the flareup.  To prevent it:

    • Use a gentle, broad-spectrum (that covers both UVA and UVB) sunscreen with sun protection factor (SPF) 30 or more daily. The sunscreen should be fragrant-free and preferably contain zinc oxide and titanium oxide, as they have the least propensity to irritate your skin.
    • If possible, avoid going out when it is sunny.
    • Always wear a wide-brimmed hat and shades whenever going out in the sun.
    • Wear loose-fitting cotton clothes for comfort.
    1. Be Stressfree

    In most individuals, stress can be the trigger for a flare-up. If it is in yours, then learn to manage stress by:

    • Being happy and doing things daily that you like and enjoy.
    • Find a hobby that helps relieve stress, like journaling, painting, doing yoga, meditation, etc.
    • Be a part of a forum or rosacea group that motivates and helps people to cope with the condition.
    • When you are extremely stressed, take a deep breath, hold it and then exhale slowly.
    1. Do not overheat your skin

    If you have flare-ups when you get over-heated, try the tips below:

    1. Do not take hot showers, instead use lukewarm water for baths and showers
    2. If the climate is cold, dress up in many layers so that if you feel overheated, you can remove some layers; if in hot climatic conditions, wear loose-fitting cotton clothes to allow air circulation and relief.
    3. Sit in places with air-conditioners or drape a wet cloth around your neck to cool off.
    4. Sit away from fireplaces, heaters or any electronic gadgets that emit heat.       
    1. Cut Down on Spicy Foods

    If eating spicy foods triggers rosacea, it is better to:

    • If your condition is still not improving, omit spices completely from your diet.
    1. Drink Cold Beverages

    Opt for cold drinks rather than hot beverages if drinking hot fluids exacerbates rosacea symptoms. Research has shown hot beverages are the main cause of rosacea flare-up in some individuals. Use these options:

    • Drink iced tea, cold coffee, cold-pressed juices, etc.
    • If the beverage is hot, allow it to cool before drinking.
    1. Be Selective While Choosing Skin Products

    If you have irritation, dryness, burning sensation or a scaly feel of the skin when applying certain skin care products, then these are signs of triggers that can cause rosacea to flare up. Try these to avoid flare-ups:

    • Stop using products that are irritating your skin.
    • When buying a skincare product, always look at the ingredients- do not use products which contain menthol, camphor or sodium lauryl sulfate, as these can irritate the skin and cause redness and flushing.
    • Use gentle skin formulas made for sensitive skin
    • Avoid the use of retinoid creams as they can trigger rosacea.
    • Do not scrub, massage or wipe your face too hard, as it causes irritation and redness.
    1. Keep A Check On Your Alcohol

    If your rosacea is triggered after drinking alcohol, then surely it’s red wine. You can still consume alcohol but:

    • Stop drinking red wine; instead, drink white.
    • If you are having a beer, whisky, wine, etc., it’s best to add soda or lemonade to decrease the amount of alcohol.

  • The Impact of Melasma on Mental Health

    Melasma is a common skin pigmentation disorder characterised by light blue, blue-grey, brown, or freckle-like patches on the skin, especially on the face (forehead, cheeks and chin) in a mask-like fashion. Melasma is derived from the Greek word “melas”, which means black Melasma is also known as the “mask of pregnancy” as it occurs mostly in pregnant women. The exact cause for melasma developing in people remains unknown, but pregnancy exposure to sunlight and ultraviolet light, family history, people on estrogen therapy and hormonal replacement therapy, and exposure to ingredients in cosmetics and phototoxic drugs play a crucial role in its development. It is seen that people who are dark-skinned, women, pregnant women and some men are affected by melasma.

    Melasma can be treated by drugs that inhibit melanin production by decreasing new melanin-producing cells and decreasing the proliferation of melanocytes. Topical hydroquinone, kojic acid, azelaic acid and tranexamic acid are effective. A mixture of hydroquinone, tretinoin and dexamethasone (Kilgman’s formula), chemical peels and laser treatment is also effective. Using sunscreen and wearing a hat while going out in the sun is important.

    How Does Melasma Impact One’s Life?

    Melasma is a preventable condition but can lead to discolouration of the skin that is difficult to treat, and the most troublesome fact is – that melasma can recur; hence, it is a cosmetic disorder that can have an emotional and psychological impact on people and can impact their quality of life. According to the World Health Organization (WHO), quality of life refers to individuals’ perceptions of their position in life within their cultural context, value systems, goals, expectations, standards, and concerns.

    Despite the treatments taken for melasma, when a patient has extensive melasma, especially on the face, the emotional and mental health of the patient may be affected. This can result in reduced social interaction,  decreased performance at work or school and low confidence and self-esteeem. 

    • People with melasma have a feeling of shame in regard to their skin , low self esteem, and try to stay confined to solitary places.     • Several studies have been conducted to find the relationship between quality of life, psychological aspects and mental health in patients with melasma. The Melasma Area and Severity Index (MASI) scale (used to evaluate the severity of melasma and the area affected by it) was used as an indicator but as people with melasma have more psychosocial than physical problems, a tool called the Melasma Quality of Life (MELASQoL) scale was developed. 
    • MELASQoL focuses on the psychosocial aspects in people affected by melasma and comprises of 10 questions, scored from 1 (not bothered at all) to 7 (bothered all the time).

    • The rating is defined as –  higher the score, the lower the quality of life.  Parameters like “Work,” “Family relationships,” “Social life,” “Sexual relationships,” “Recreation leisure,” “Physical health,” “Money matters,” and “Emotional well-being.”  were included to find out the impact of melasma on the psychosocial aspect of people. 

    Studies involving patients with melasma were done in  most countries using MELASQoL and it was seen that:

     In patients with melasma, emotional distress and decreased social interaction was prevalent. 

    People with melasma showed dissatisfaction, frustration, embarrassment, and depression related to their skin condition. 

    They feel that the patches make them feel unattractive, which affects their social life- lack of desire their desire to be with people or interact with them. This leads to decreased quality of life.  

    However, people with melasma were affectionate towards others.

    Treatment of melasma can only relieve the symptoms for some time in a few people, but at times after the treatments, melasma can flare up and cause discomfort.   • Will Retinol Minimize Pores

    Pores are tiny openings at your skin’s surface that allow the skin to breathe. Pores are the opening of your hair follicles on the skin. Oil or sebum is carried from the deeper layer of the skin to the surface by ducts and opens onto the skin through the pores. In normal skin, your pores are tightly bound, leaving your skin youthful, firm and healthy. As we grow old, there is only shedding of our skin cells and the dead skin, debris and sebum present on the surface gets trapped inside the pores. As a result, the skin expands, the pores open, and they appear enlarged (more than their actual size).

    These pores need to be minimised when they become enlarged and visible because they cause the skin texture and appearance to dull and dry. Clogged pores also affect collagen production, leading to skin elasticity loss, making the skin look aged, wrinkled, and sheenless.

    What is Retinol And How Does it Work?

    Retinol is the commercial name for retinoid – a class of drugs formed from Vitamin A. In short, retinol is the formulation of Vitamin A.     Our pores regulate the skin’s temperature (by producing sweat to cool off when it is hot), keep the skin’s surface moist (by producing oil/sebum from sebaceous glands) and protect us against external bacteria and allergens.

    The pores on our skin are lined by a protein, keratin (present in the skin, nails and hair) that helps to bind the cells together. If keratin production increases inside the skin (hyper keratinisation) due to any reasons (hormonal changes, improper cleaning of the skin, use of products that are harmful to the skin, etc.), the normal shedding of the skin is interrupted due to the excess keratin.

    As a result, the excess keratin combines with the sebum, causing the skin cells to coalesce together, forming a keratin plug (contains oil, keratin and debris) – as a result, there is clogging of the pores, leading to enlargement of the pores.

    Retinol acts on the clogged pores and increases the speed of cell turnover (making your pores appear smaller by increasing collagen production, thereby making your skin thicker). Retinol by its exfoliation action on the skin improves your skin’s texture and tone.

    Using Retinol

    Retinol is generally safe to use and has very few side effects. You can start using it when you are 20 years old or during puberty (as per the advice of your dermatologist). Retinol comes in the form of creams, lotions, and serums. You can start using it for your pores 2 to 3 times a week and then make it a part of your daily evening routine use. This should be done after consulting with your dermatologist.

    Side Effects of Retinol

    As with every product, Retinol has its side effects, which are present initially – dry and irritated skin, redness, itching and peeling. These will disappear after continuous use. If the irritation persists, then it is advised to consult your dermatologist.

  • Q- switch Laser Treatment – What is it?

    Nowadays, lasers are used for many procedures. Laser means “Light Amplification by Stimulated emission of Radiation”. Laser treatment is used for acne, age spots, unwanted hair on the body, unwanted tattoos, broken blood vessels, etc. Lasers emit high-intensity light with a single wavelength (with one colour) and target a specific tissue known as chromatophore (part of the skin tissue that gives it the colour) in the skin. The chromatophores absorb the rays and get destroyed. This causes the formation of new cells by stimulating collagen production, replacing the damaged cells, resulting in a clear, smooth and supple skin. Q-switch laser, previously a gold standard treatment for tattoo removal, is an important tool in cosmetic dermatology.

    What does Q-switch Laser Treat?

    The Q-switch laser is non-ablative and can heat the deeper layers of the skin without damaging the surrounding tissues. This also prevents the development of scars. Q-switch laser is used for:

    Different brands of Q-switch lasers exist in the market – Q-switched Nd: YAG lasers, Q-switched Ruby lasers, Q-switched Alexandrite lasers and Picosecond Alexandrite lasers.

    How Are Q- switch Lasers Different From Regular Lasers?

     Q-switch lasers deliver the energy (light) in very short pulses of the same wavelength. These very short pulses of energy disrupt the pigment-containing cells leading to their destruction. This property of Q-switch lasers makes them beneficial in treating hyperpigmentation issues and tattoo removal. Regular lasers, on the other hand,  emit energy in one pulse (one continuous light beam) to the targetted area, commonly water, pigment cells and blood vessels. The advantage of using a Q-switched laser is that as the energy is emitted in bursts, it allows time for precision and minimises the heat built up, thereby reducing damage to the tissue present beneath the skin.

    Q-Switch Laser Treatment

    This non-ablative treatment will take around 40 to 45 minutes per session. Your surgeon will inform you on the number of sessions required depending on your condition.

    What to Expect Before Your Laser Treatment

    • Your doctor will record your detailed medical and family history for any debilitating diseases (like diabetes, high blood pressure). You should inform your dermatologist about any allergies that you have or supplements that you are on.
    • Your dermatologist will ask questions about your daily skin routine and conduct a thorough physical examination.
    • Your dermatologist will discuss your option and recommend a course (number of sessions required) for the treatment.

    What to Expect On the Day of The Treatment?

    Your dermatologist will clean the treatment area thoroughly and apply a local anaesthetic cream to numb the area.

    A thin layer of carbon cream is applied, which helps break down pigment cells in the skin by the laser beam.

    The laser beam of a particular wavelength (relative to your condition) is applied in bursts on the skin- this will cause the heating up of the carbon and stimulates its binding to the skin.

    Finally, the laser will break the liquid carbon into small particles removed by the smoke evaporator connected to the laser. The debris along with impurities in the carbon particles, are removed by the evaporator.

    Once the session is completed, cotton balls dipped in saline is used to clean the skin.

    How is The Recovery Period After the Session?

    There is no downtime associated with Q-switch Laser, and you can resume going to work the same day. In some cases , the skin may remain grey or white for a few minutes. In individuals with sensitive skin, redness may persists for a few days. You should not be worried as it will subside on its own. Within 7 to 10 days, the skin is healed. The time between sessions will depend on your condition and skin type. Some of the precautions to be taken after the session:

    • Do not go out in the sun
    • Do not soak the treated area in water
    • Do not touch, itch or prick at the treated area
